With Intake Forms 2.0, Direct Primary Care clinicians can now customize intake form questions and modify the mapping of the data to meet the clinician’s needs or workflows. This allows you to tailor the questions asked of members when they join your practice or come in for regular visits, and craft highly personalized care plans based on the fields included on your forms.

  1. You can send a member form via their chart > + Interactions > Forms > Choose a form from the dropdown menu (this will ensure the form is submitted to the patient's chart)

  2. When the patient submits the form, open it in the chart interaction and click 'Add to Health History'

  3. Next, you will choose where each field maps to the member's chart

  4. Click 'Confirm'

  5. Navigate to the chart and the information will be added to the selected fields

Helpful Tips:

  • Where possible, use open-ended questions when building forms

  • Add the template form called “General Intake Form (Hint Template) - Please Customize”

    1. Forms > Forms > Standard Forms > General Intake Form (Hint Template) - Please Customize > Import

    2. Customize the form for your practice

  • Old submissions can also be mapped

Q: How can I access the mapping tool for submitted intake forms?

A: The mapping tool can be accessed through Communications > Forms in Hint Clinical, or by opening the member’s interaction list and filtering for forms.

Q: Can I customize where questions from a submitted form map in the Health History section?

A: Absolutely! You have the flexibility to select specific questions and map them to any section in the health history for the member, ensuring a tailored approach.

Q: Is there a new section introduced in the health history?

A: Yes, a new section called "General Info" has been added to the health history. This section allows for easy mapping of Intake items and provides a pinnable feature for added scannability of important information.

Q: How can I map a completed form to a member with no associated profile?

A: If a form is received via email address, you can create a new profile for the member and decide to map the form to this or any other member, providing flexibility in data management.

Q: Is there a data type that does not map properly to member charts?

A: Yes, if there is a matrix or body map, the field you map the data to will show a link to open the intake form. Matrixes and body maps have complex data types that prevent them from pulling in to the field
