How to create a consent form

Consent forms allow for a health care provider to educate a patient about the risks, benefits, and alternatives of a given procedure or intervention. Consent forms are typically provided for a patient to sign off on a procedure, giving consent.

To create a consent form in Hint:

  1. Navigate to Hint Clinical

  2. Click on Forms

  3. Within the Forms page, click on 'Forms' (at the top of the page)

  4. Under Consent Forms, click 'Create New'

  5. Input a Form Name

  6. Select either the Rich-text Template (to create a consent form, similar to editing a Word document) or a Native PDF option (to upload a PDF file that you already have created)

  7. Click Continue to start editing your consent form

Capabilities within Consent Forms include:

  • Editing Form Name

  • Enabling an e-signature

  • Enabling a provider e-signature

  • Enabling an assistant e-signature (Witness)

  • Making a signature option

  • Disabling the form header

Additionally, on the left-hand side of the screen, you have the following options:

  • Send to Client - This sends the consent form to the client/patient of your choice

  • Preview - Allows for you to see a preview of the consent form

  • Open PDF - This opens your consent form as a PDF

  • Add to Intake - This allows for you to add your consent form to any intake forms you have already created

  • Get URL - Provides you with the URL to embed in your website, provide to patients externally, etc.

  • Duplicate - This action will duplicate the consent form

  • Delete - The consent form automatically saves while you are working on it. To delete the form, you will click this button
